Community Visiting Nurse Association has served the residents of Somerset and Middlesex Counties since 1948 and expanded into Warren County, NJ, in October, 2014. Our caring, knowledgeable professionals come to your home or place of residence to provide you with top quality home care based on your needs. We can also connect you to appropriate community services for other types of support. We have expertise in areas such as disease management, cardiac care, wound care, geriatrics, post-surgical care, diabetic care, maternal/child care and pediatrics, as well as others. You can call on us for:You may qualify for Community VNA home care if you are a resident of Somerset, Middlesex or Warren County, if your physician has ordered home care and if you require skilled professional care (such as nursing care, teaching or rehabilitation services). Our services may be covered by Medicare, Medicaid, managed care, private insurance or on a private pay/sliding scale basis. You do not need to be hospitalized to qualify to receive home care services from Community VNA. To get started, just call our Intake Department (908-725-9355, Ext. 2222). - Your case manager or health care professional
- A community agency
- A coordinator in a hospital or nursing facility
- An assisted living or rehabilitation facility
When we receive your call for service, we’ll contact your physician and set up a visit with you to determine how best we can help. From the beginning, we involve you in your plan of care, and, when appropriate, teach you, your family members and/or caregivers about your treatments or therapies. We schedule regular visits by our staff to help you get the care you need and stay as independent as possible in the comfort of your own home or residence. A staff member is on call 24 hours a day, 7 days a week. Community Visiting Nurse Association is certified by the Centers for Medicare and Medicaid Services (CMS), licensed by the State of New Jersey Department of Health and accredited by Community Health Accreditation Program (CHAP).
