Interdisciplinary Team

Community Care Hospice provides care through an interdisciplinary team approach, using many resources to help you. Working with your physician and our medical director, our team of registered nurses, social workers, chaplain, home health aides, nutritionists, therapists and volunteers will support you in comfort and care during regular visits. We will also provide you and your loved ones with education, as well as emotional and spiritual support. We will come to your home (or place of residence) and create a plan to maximize comfort for you. Our team works together to make certain that all of your needs are met. Community Care Hospice is available in your home, in assisted living facilities as well as skilled nursing facilities.
