At Community Visiting Nurse Association, quality
of care is a major focus. An integral part of Quality Improvement
involves OBQI, or Outcome Based Quality Improvement. Through the
OBQI process, we can evaluate data and statistics and benchmark
our agency with other agencies in the region as well as nationally.

We are continuously
analyzing our data which is reported through Home Care Compare and OCS (Outcome Concept Systems). This
process enables us to benchmark our data against other regional and national home care providers. It allows us to monitor
our quality of care. Some of the outcomes that are analyzed are:
- Percentage of patients who get better at
walking or moving around
- Percentage of patients who have less pain
when moving around
- Percentage of patients who stay the same
(don’t get worse) at bathing
- Percentage of patients who had to be admitted
to the hospital
- Percentage of patients who need urgent,
unplanned medical care
In
keeping with Community Visiting Nurse Association's commitment to provide a safe environment for our clients, the agency has instituted two programs that have been very successful, the Falling
Star Program and the Fire Safety
Program.
The Falling Star
Program identifies clients at risk for falls during their initial
assessment. If the client is identified to be at risk, the nurse
works with him/her and the care giver in putting safety measures
in place in the home.
The Fire Safety Program
was developed to identify clients
who do not have working smoke detectors in their homes.
