Under
the direction of your physician, a Registered Nurse will visit
you and provide a thorough assessment. As a Case Manager, she will work with you to meet your goals, provide hands-on
care, assessment, teaching, support and identify other services as needed.
Our nurses have expertise in many areas including:
- disease management
- medication management
- diabetic care
- cardiac care
- wound care
- post surgical care
- geriatrics
- maternal/child care
- pediatrics
Medical Social
Services
If appropriate, a licensed Social Worker will visit
to assist with your psycho-social needs including referrals
to appropriate community services (Meals on Wheels, county social
services, etc.), help finding an adult day care program and
assistance with long term care placement.
Certified Home Health
Aides
Home Health Aide care
may be provided in conjunction with skilled care. A Certified
Home Health Aide will visit (with your doctor’s order)
to assist with activities of daily living ,including personal
care, bathing, grooming, dressing, meal preparation, personal
laundry and food shopping. A Registered Nurse supervises all
Home Health Aides.
Nutritional Counseling
A Registered Nutritionist
will visit to provide education regarding special diet needs,
working with you to find foods you like that are within your
restrictions. She will teach you how to choose, shop for and
prepare foods that are on your special diet.
