Let us introduce ourselves. We are Community Visiting Nurse Association, serving Somerset and Middlesex counties. We are caring, knowledgeable staff who come to your home to assist you and your family when you are recovering from an illness or injury. Our goal is to provide you quality home care that includes a variety of health services, all based on your needs.

We work closely with you and your physician to plan for and provide the services you need to get better at home. Our expert staff of nurses, physical, occupational and speech therapists are all available to you whenever you need us. We offer disease management, medication management, wound care, medical social services, nutritional counseling, maternal/child health care as well as pediatric care. In addition, we are able to provide home health aides to assist you with personal, non-medical care. We will show you how to obtain and use any equipment or supplies your doctor may have prescribed and we will direct you to appropriate community services for other types of support.

Do you qualify for Community VNA home care? You do if you are a resident of Somerset or Middlesex counties, your physician has ordered home care and you require skilled professional care, such as, nursing care, teaching or rehabilitation services. Services may be covered by Medicare, Medicaid, managed care, private insurances or on a private pay/sliding scale basis. You do not need to be hospitalized to qualify for Community VNA home care services. All we need is a phone call (908-725-9355 ext. 2222) from:

• You or your family
• Your physician
• Your case manager or health care professional
• A community agency
• A coordinator in a hospital or nursing facility
• An assisted living or rehabilitation facility

When we receive a call for service, we'll contact your physician and promptly set up a visit, so we can determine how we can help you. From the beginning, we involve you in your plan of care and, where appropriate, teach you, family members or caregivers treatments or therapies. We'll schedule your regular visits by our staff. We are on call and available to you 24 hours a day, 7 days a week. Your goal is to be well and independent. Our goal is to get you there, in the comfort of your own home or residence.
